turned into Mr. Norton in order for you to participate in Teague Middle
School Athletics.
Middle School Sports Screening/Physical Form
Sports Physical Addendum
Medical Release and Consent- Athletics
Please provide a hard copy to the school of the medical release and consent form to the school.
